Product Description:

Cloran is an antibiotic containing Cefaclor Monohydrate, a second-generation cephalosporin antibiotic that is effective against a wide range of bacterial infections. It is formulated as a powder for suspension and is reconstituted with water before use, offering a convenient liquid form for children and adults who may have difficulty swallowing tablets.

  • Each 5 ml of the reconstituted suspension contains:

    • Cefaclor Monohydrate 250mg (equivalent to 200mg of Cefaclor)

Cefaclor works by inhibiting the bacterial cell wall synthesis, leading to bacterial cell death. It is primarily used for treating infections caused by susceptible bacteria.


Indications:

Cloran (Cefaclor Monohydrate) is used for the treatment of bacterial infections caused by organisms susceptible to Cefaclor. It is effective in the treatment of the following conditions:

  1. Respiratory Tract Infections:

    • Acute otitis media (middle ear infections)

    • Sinusitis

    • Pharyngitis (inflammation of the throat)

    • Tonsillitis

    • Bronchitis

    • Pneumonia

  2. Urinary Tract Infections (UTIs):

    • Cystitis (bladder infection)

    • Pyelonephritis (kidney infection)

  3. Skin and Soft Tissue Infections:

    • Cellulitis

    • Abscesses

  4. Other Infections:

    • Dental infections

    • Gonorrhea (when caused by susceptible strains)


Dosage:

The dosage of Cloran (Cefaclor Monohydrate) depends on the type and severity of the infection, as well as the patient’s age and renal function. The general recommended dosages are as follows:

  • Adults and Children over 12 years:

    • 250 mg every 8 hours, or 500 mg every 12 hours, depending on the severity of the infection.

    • In severe infections, the dosage may be increased to 500 mg every 8 hours.

  • Children (under 12 years):

    • Children aged 1 month to 5 years: 125 mg every 8 hours, which can be adjusted depending on the severity of the infection.

    • Children aged 6 to 12 years: 250 mg every 8 hours.

  • For pediatric dosing (oral suspension):

    • Reconstitution: Mix the powder with water as per the instructions to prepare the suspension.

    • The usual dose for children depends on their weight, with the typical dose ranging from 20 to 40 mg/kg/day divided into three doses.

    • For instance:

      • Mild to moderate infections: 20 mg/kg/day

      • Severe infections: 40 mg/kg/day

The dose should be adjusted based on the severity of the infection and the child’s weight.


Side Effects:

Cloran (Cefaclor) may cause side effects, although not everyone experiences them. Common side effects include:

  1. Gastrointestinal:

    • Diarrhea

    • Nausea

    • Vomiting

    • Abdominal pain

    • Flatulence (gas)

  2. Allergic Reactions:

    • Rash

    • Urticaria (hives)

    • Angioedema (swelling, especially of the face or throat)

Contraindications:

Cloran (Cefaclor) should not be used in the following situations:

  1. Allergy to Cephalosporins:

    • Cloran is contraindicated in individuals with a known hypersensitivity (allergy) to Cefaclor, cephalosporins, or any of the components of the product.

  2. Severe Renal Impairment:

    • In patients with severe renal failure or significant renal impairment, Cefaclor should be used cautiously, and dosage adjustments may be necessary.

  3. History of Colitis:

    • Cloran should be used with caution in patients with a history of colitis, particularly antibiotic-associated colitis caused by Clostridium difficile.

  4. Mononucleosis:

    • Cefaclor should not be used in patients with mononucleosis (viral infection) due to the increased risk of rash.

  5. Liver Dysfunction:

    • Care should be taken in patients with liver dysfunction, as Cefaclor can cause liver enzyme elevation.


Precautions:

  • Superinfection: Long-term use of Cefaclor may lead to an overgrowth of resistant organisms or fungi (e.g., Candida), leading to secondary infections.

  • Renal Dysfunction: Patients with renal impairment may require dosage adjustments. Monitoring of renal function during treatment is advised.

  • Clostridium difficile-associated Diarrhea (CDAD): Antibiotic treatment, including Cefaclor, can lead to an overgrowth of C. difficile, which may cause severe diarrhea. Discontinue use if severe diarrhea develops.

  • Pregnancy: Cefaclor is generally considered safe during pregnancy (Category B), but it should only be used when necessary and under medical supervision.

  • Breastfeeding: Cefaclor is excreted in breast milk in small amounts and is generally considered safe for use during breastfeeding.
